Integris Health Data Breach Settlement 2025: Submit Claim To Get Up To $25,000!

Integris Health has agreed to the settlement of $30 million, which is all set to be distributed among the class members, i.e., those who were affected by the data breach that took place in November 2023. Here, we have discussed eligibility, payout, claim process, and much more.

This settlement resulted from a class action lawsuit, which was filed in July 2025 against the company alleging that it doesn’t take appropriate measures to protect personal information of patients, current and former employees, and minors.

After the lawsuit, the company agreed to a settlement in October 2025 without accepting any wrongdoing, and then it got the preliminary approval. Then the settlement portal was made public for the class members to opt out, object, and submit the claim form.

Note that the deadline to opt out from the class members and object to the settlement is November 21, 2025, and the due date to submit the claim form is December 22, 2025. The hearing for the final approval is on December 16, 2025, and then the payout will be sent out after the final approval.

When are we eligible to get a payout from the Integris Health Data Breach Settlement?

To get a payout from the Integris Health Data Breach class action settlement, it is mandatory that one be among the class members. The eligibility for the same is

  • You must be a permanent resident of the United States of America and live in any territory of the country. 
  • Your personal information, such as names, addresses, dates of birth, Social Security numbers, health insurance information, and some medical information, was compromised in the November 2023 data breach.
  • You must have received a notice from the settlement administrator about the Integris Health Data Breach Settlement, which must carry a unique ID and PIN.

If you haven’t received any notice about the settlement, but you believe that you are among those whose personal information was compromised during the data breach, you can mail a claim form with required documents.

How to submit a claim form for the Integris Health Settlement?

Online:

  1. Go to the Integris Health Data Breach Settlement official website at integrisdataincidentsettlement.com/.
  2. Click on the ‘SUBMIT A CLAIM’ button and get redirected to the next page.
  3. Enter your Unique ID and PIN, and then click the Login button.
  4. Fill in the claim form by providing all the required details and attaching documents.
  5. Choose the payment gateway to get payout from the settlement fund, and submit the claim form.

Offline:

  • Download the PDF format of the Integris Health Data Breach Settlement claim from the official website.
  • Fill in all the details accurately using a blue or black pen, and then attach documents, if you have any.
  • Mail it to Integris Data Security Incident Settlement, c/o Settlement Administrator, P.O. Box 25205, Santa Ana, CA 92799 by December 22, 2025.

How much will be received from the Integris Health Settlement?

By submitting a claim form for the Integris Health Data Breach class action settlement, you can get up to $25,000. The amount is going to be received by those individuals who will provide documentation for the out-of-pocket expenses incurred due to the data breach.

If you don’t provide documents with your Integris Health Settlement claim form, you will get payment on a pro rata basis. The maximum you can get is $100. Note that the payment will be received by you through the payment method you have opted for in your form. 

When to expect the Integris Health Settlement payout?

The payout for the Integris Health data breach settlement will begin after the final approval. Note that if the settlement receives final approval without any delay, the payment will be received within 90 days. In case of the appeal, the payment may be delayed unexpectedly.

If the settlement receives final approval from the court on December 22, 2025, without any appeal, it is highly expected that the payment will be received in March 2026. It is approved with appeal; we may have to wait until the last quarter of 2026 for the payout.
